Back Pain Is One of the Most Common Auto Injury Symptoms — and One of the Most Undertreated

When people think about car accident injuries, whiplash and neck pain tend to come to mind first. But back pain following a car accident is just as common — and in many ways just as consequential. The lumbar spine absorbs significant force in a collision, and the injuries that result can range from mild muscle strain to serious disc and joint damage that, if left untreated, becomes a long-term problem.

At Nowak Chiropractic in South Buffalo, Dr. John Nowak has been treating back pain from auto injuries for over 40 years. He has seen every presentation — the patient who walked away from the accident feeling fine and showed up two weeks later barely able to stand, the patient whose lower back has been tight and achy since a rear-end collision six months ago, and everything in between.

The pattern he has observed consistently is that early, proper treatment produces the best outcomes. And that most patients who end up with chronic back pain from an accident never received proper treatment in the first place.

How a Car Accident Injures the Lower Back

The lumbar spine is a strong and resilient structure under normal conditions. But a car accident is not a normal condition. The forces involved in even a low-speed collision can exceed what the lumbar spine was designed to handle — particularly when those forces are applied suddenly and from an unexpected direction.

In a rear-end collision, the lower back is compressed and then extended rapidly as the body is driven forward and then restrained by the seatbelt. The lumbar discs, facet joints, and the muscles and ligaments that support the spine can all be injured in that sequence. In side-impact collisions, the lateral forces applied to the spine create a different but equally damaging pattern of joint compression and soft tissue strain.

The seat itself plays a role. A vehicle seat that transfers force efficiently into the occupant’s lower back — particularly in rear-end impacts — can concentrate the collision force directly into the lumbar spine in a way that produces significant injury even when the visible damage to the vehicle is minimal. Low-speed collisions with little vehicle damage are not necessarily low-injury collisions. The human spine does not work that way.

Why Back Pain From an Accident Is Different From Ordinary Back Pain

Back pain that develops after a car accident has a specific injury pattern behind it that distinguishes it from the back pain that develops from poor posture, overexertion, or gradual degeneration. The structures that were stressed or damaged in the collision are specific. The way the pain behaves — how it changes with position, what aggravates it, whether it radiates — reflects that specific injury pattern.

This matters because the treatment that works for ordinary mechanical back pain is not always the right approach for post-accident back pain. The injured tissues need to be identified specifically. The extent of disc involvement, if any, needs to be assessed. The nerve involvement that produces radiating leg pain or sciatica in some accident patients needs to be recognized and addressed appropriately.

Dr. John’s approach to post-accident back pain begins with exactly that kind of specific evaluation. He examines the lumbar spine hands-on, assesses what is actually happening in the injured structures, and develops a treatment approach based on what he finds — not a generic back pain protocol applied uniformly to every patient.

Back Pain Symptoms That Can Develop After an Auto Injury

  • Local lower back pain at the site of the injury

  • Muscle spasm and guarding in the lumbar and paraspinal muscles

  • Stiffness and reduced range of motion in the lower back

  • Pain that worsens with sitting, particularly for extended periods

  • Radiating pain into the buttock or leg from lumbar nerve involvement

  • Sacroiliac joint pain — deep, one-sided pain in the lower back or pelvis

  • Pain that is worse in the morning and takes time to loosen up

  • Disc-related symptoms including sharp pain with certain movements

Your No-Fault Insurance Covers Back Pain Treatment

If your back pain developed from a car accident in New York State, your no-fault auto insurance covers chiropractic care — including the treatment of lower back injuries — regardless of who caused the accident. You do not need to wait for a settlement, prove liability, or pay out of pocket for covered treatment.

The critical detail is timing. New York no-fault law requires that you seek treatment within 30 days of the accident to preserve your benefits. If you have been delaying care hoping the back pain will resolve on its own, that window may be closing. Do not wait until it has closed.

Dr. John has been treating no-fault patients and navigating the no-fault insurance system on their behalf for over 40 years. He understands the documentation requirements, the IME process, and how to protect his patients’ claims while delivering the clinical care they need to actually recover.
