Sciatica is one of the most recognizable pain patterns a person can experience. The sharp, burning, or shooting sensation that travels from the lower back through the buttock and down the back of the leg along the path of the sciatic nerve is unmistakable. It can make sitting unbearable, standing exhausting, and finding a comfortable position at night nearly impossible.


What makes sciatica particularly frustrating is that the pain is in the leg, but the problem is not. The sciatic nerve is being compressed or irritated somewhere along its origin — most often in the lumbar spine or the deep muscles of the hip — and the pain travels down the nerve’s path to wherever it is felt most intensely. Treating the leg does nothing. The source has to be found and addressed, and that requires a provider who knows where to look.

What Is Actually Causing Your Sciatica


Sciatica is a symptom, not a diagnosis. The pain in the leg is the signal that something is compressing or irritating the sciatic nerve or the lumbar nerve roots from which it originates.


The most common mechanical causes include:


Lumbar Disc Herniation


A disc that has bulged or herniated in the lower lumbar spine can press directly on the nerve root that feeds into the sciatic nerve. This is the most common cause of true sciatica and produces the classic shooting pain down the leg along with numbness, tingling, and sometimes weakness in the foot or lower leg. Many lumbar disc herniations that are producing sciatic symptoms respond well to conservative chiropractic care, particularly when treatment is sought before the condition has become chronic.


Lumbar Joint Dysfunction


Restricted or misaligned lumbar vertebrae can narrow the opening through which the nerve root exits the spinal canal, irritating the nerve without a disc herniation being present. This type of sciatica is highly responsive to chiropractic adjustment because the mechanical restriction causing the nerve irritation is directly addressable with spinal manipulation.


Piriformis Syndrome


The piriformis muscle sits deep in the buttock, directly over the path of the sciatic nerve. When this muscle becomes tight, spasmed, or inflamed, it can compress the sciatic nerve against the surrounding bone and produce a pain pattern that is identical to lumbar-originated sciatica. Piriformis syndrome is frequently overlooked because it does not show up on spinal imaging and requires a hands-on clinical examination of the hip musculature to identify.


Why Sciatica Often Gets Worse Before It Is Treated


One of the patterns Dr. John sees consistently in sciatica patients is that many of them waited far too long before seeking treatment. The leg pain started, they rested, it improved somewhat, they resumed activity, it got worse, they rested again. Each cycle of flare and partial recovery left the underlying mechanical problem a little more established and the recovery a little harder to achieve.


Sciatica that has been present for months is more complex to treat than sciatica addressed in the first few weeks. The nerve that has been irritated for an extended period becomes sensitized in ways that prolong recovery even after the mechanical source of compression is corrected. The muscles that have been compensating around the restricted or herniated structure become increasingly entrenched in their guarding patterns. Early treatment consistently produces better outcomes than waiting.


Sciatica Symptoms That Chiropractic Addresses


  • Shooting, burning, or sharp pain from the lower back or buttock down the leg

  • Numbness or tingling in the leg, foot, or toes

  • Pain that is worse with sitting, particularly for extended periods

  • Pain that worsens when rising from a seated position

Dr. John assesses range of motion, identifies areas of restriction, and evaluates the nerve symptom pattern to determine what is most likely driving the sciatica. Treatment is tailored to what he finds — not a standard protocol applied to every patient who walks in with leg pain.


For disc-related sciatica, adjustments are applied with the specific goal of reducing pressure on the affected nerve root and restoring normal movement to the lumbar segments surrounding the disc. For joint dysfunction-related sciatica, targeted adjustments correct the restriction that is narrowing the nerve exit. For piriformis-related presentations, treatment addresses the deep hip musculature contributing to the nerve compression.


Most sciatica patients notice a reduction in leg pain intensity fairly early in the course of care — often before the lower back pain that may be accompanying it fully resolves. The leg pain retreating back toward its source is a positive sign that the nerve irritation is decreasing. As care continues, the mechanical problem at the root of the sciatica is progressively corrected and the leg symptoms continue to reduce.


A Note on Serious Sciatica Symptoms


Most sciatica is mechanical and responds well to conservative chiropractic care. However, some presentations require urgent medical evaluation rather than chiropractic treatment. Loss of bladder or bowel control, severe and progressive leg weakness, or numbness in the groin or inner thigh are red flag symptoms that require immediate emergency care.
